Service Learning Forms & Tools

Steps to Service Learning
2010-2011 Academic Year

If you have never done a Service Learning project before:

1. You must schedule an appointment to meet with Director of Experiential Learning Opportunities & Civic Engagement, Fagan Forhan. You can call ext. 595 to make an appointment or stop by room 279 (2nd floor crossover).

If you have done a service learning project in the past:

1. Obtain required paperwork. This includes the following:
  • If you are doing a class project or have students working on campus, you need On-Campus Contracts and time sheets;
  • If you are having students go off‐campus, you need Off-Campus Contracts, Assumption of Risk Forms, CORI forms, time sheets and Agency Evaluation Forms

You can obtain hard copies of these forms through Kathy Laperriere in the Academic Affairs office, room 118.
2. If you need to find site placements, want to brainstorm about project integration or would like to request an in-class presentation, contact Fagan Forhan at ext. 595 or in room 279 in the 2nd floor crossover.

3. Make certain that students have completed the Service Learning Contract and if they are doing an off‐site placement, the CORI form and Assumption of Risk Forms prior the start of the project. This is mandatory and they must be submitted to room 118 prior to the start of any service learning placement or project.

4. All hours must be reported by students via a time sheet or via the online reporting system by April 30th. All hours reported through the online mechanism will be sent to sponsoring faculty members for approval and verification the week of May 1st. Updates are also available to faculty at any time by request. Once hours are approved by their sponsoring

Reporting Hours & Activities

Students are now responsible for reporting their own hours and activities.
This information is collected and reported to the Corporation for National and Community Service, and has helped MWCC obtain the Carnegie Classification in Community Engagement and Outreach and Partnerships as well as earning the Presidents Community Service Honor Roll each year since its inception in 2006. The data is also used to assist in leveraging funding and support for new civic engagement and service learning initiatives.

Hours and activities should be reported via our online tool, which is also under "quick links" in the iConnect student portal.

If you would like a brief classroom presentation or have questions on how to use the online log, please contact Fagan Forhan at x-595 to schedule.
